Life is full of changes – whether it’s a new marriage, the joy of a child or a shift in employment (to name a few).  We’re here to support you through all of it with a benefits package designed to keep you covered financially, health-wise, and in planning for the future.

Did you know that major life changes, which in the benefits world are called  “qualified life events”, allow you to make changes to your benefits due to your life change? (for example: when you get married, you can add your spouse to your medical plan)

Some common qualifying life events include:

  •  Marriage, divorce, or legal separation
  • Birth or adoption of a child (or placement of adoption)
  • Loss of a covered dependent, including spouse
  • Change in insurance eligibility (a new job, loss of job, relocation, etc.)

When a qualifying event happens, you have a 30-day window to make changes to your benefits.  To get started, or if you have any questions, contact HR so we can help you determine if a change is needed. Because these changes are governed by the IRS, you are unable make changes after the 30-day window.  This means any changes will need to wait until our next Benefits Open Enrollment.  Information regarding Benefits Open Enrollment is communicated throughout the year.  Keep an eye out for updates!

Marriage Checklist

Reminder

You must provide a copy of your marriage certificate to change ANY benefit. 

Your deadline to enroll or make changes is within 30 days of the marriage.

Medical, Dental, and Vision Plans
What to consider 

  • Compare your and your new spouse's coverage options.
  • Compare your potential out-of-pocket costs: premiums, deductibles, copays and coinsurance.
  • Compare prescription drug plans. Are medications you take covered at a higher level under one plan?

What you can do

  • Enroll yourself, your new spouse and any eligible dependent children within 30 days of your date of marriage. Coverage is usually effective the date of the marriage. Note: If you are currently covering your dependents and want to add your new spouse's dependents, you may. However, you must provide a birth certificate for the child(ren) with a different last name. 
  • Cancel your coverage if you enroll in your new spouse's coverage. Coverage usually ends at the end of the month in which you cancel your coverage.

Flexible Spending Accounts

  • Make sure you compare the available reimbursement accounts and select the plan(s) that best fits your needs.
  • Check out www.irs.gov for a list of eligible expenses.

Optional Life Insurance and Voluntary AD&D

  • Determine the coverage levels for your new family, considering future plans and both long-term and short-term debt.
  • Make sure you update your beneficiary designation as well.

Retirement Benefits

  • Make any investment changes that meet your new needs for your family.
  • Make sure you update your beneficiary information.
  • Remember to notify your plan provider of any change to your personal information.

Employee Information

  • Always make sure your personal information stays current. Update the following information:
    • Change name and address
    • Marital status
    • Tax withholding information
    • Emergency contact information
Divorce Checklist

Reminder

  • You must present a copy of your divorce decree for all benefits changes.
  • You must make changes in coverage within 30 days of the official date of divorce.

Medical, Dental, and Vision Plans
Changes to consider after the divorce is official:

  • Remove ex-spouse and ex-step children from health benefits.
  • Get the address of your ex-spouse for COBRA continuation notification.
  • If coverage from ex-spouse was lost, enroll in company benefits within 30 days.

  • Make changes to any reimbursement accounts depending on your new expenses.
  • Check out www.irs.gov for a list of eligible expenses.

Optional Life Insurance and Voluntary AD&D

  • Determine your new coverage requirements or make changes to your policy to reflect your new status.
  • Remember to update your beneficiary information.
  • Don't forget to update your will or trust.

Retirement Benefits

  • Remember to update your beneficiary information.
  • Notify plan provider of any change of address.

Child Birth & Adoption Checklist

Reminder

  • Notify your plan provider of the upcoming birth OR adoption of your child to determine if there are any pre-approval requirements.
  • If you elect to enroll your new child in our benefits plan, you must do so within 30 days of your date of birth or adoption.

  • What to consider in choosing which health benefits to choose for your new child:
    • Compare both parent's plans, keeping in mind well-child care and immunizations.
    • Check with your pediatrician to determine which plans they accept.

Dependent Status Change Checklist

Medical, Dental, and Vision Plans

  • Notify plan administrator of change within 30 days for COBRA continuation coverage.

Flexible Spending Accounts

  • Check policy for continuing account coverage for expenses.
  • Notify of changes within 30 days of change.

Dependent Life Insurance

  • Coverage ends on the last day of the month in which the change occurs.

Tax Liabilities

  • Update tax withholding information if change impacts the number of dependents.
Death Checklist

In Case of Employee's Death

Medical, Dental, and Vision Plans

  • Your spouse needs to check with the Benefits Department to determine continuation of benefits and their costs.

Life Insurance

  • You must present a copy of the official Death Certificate for claims processing.
  • If you have a question about the deceased's coverage, contact the Benefits Department.

Retirement Plans

  • Contact the Benefits Department with questions and contact information for plan providers.

Loss of a Family Member Checklist

Medical, Dental, and Vision Plans

  • Contact the Benefits Department for assistance to end coverage of deceased.

Life Insurance

  • You must present a copy of the official Death Certificate for claims processing.
  • Remember to make changes to benefits and beneficiary information.

Retirement Plans

  • If necessary, change beneficiary information.

 

Leaving the Company Checklist

Notification of Departure

  • Please notify the Benefits Department

Medical, Dental, and Vision Plans

  • Determine need for COBRA continuation, which is available for up to 18 months from last date of the month in which you terminate.
  • Ask for the cost of the COBRA premium.
  • Check mail for notification of COBRA continuation coverage.

Flexible Spending Accounts

  • You may continue to make contributions on an after-tax basis through the end of the plan year.
  • Ask about the process for submitting claims for reimbursement.
  • Check out www.irs.gov for a list of eligible expenses.

Life Insurance

  • Evaluate the needs of your family and purchase or upgrade any life insurance policies.
  • Update beneficiary information if needed.

Retirement Benefits

  • Update your change of information with the plan provider.
  • Ask about the process for loan repayment.
  • Determine if you are eligible to keep your vested balance with the plan.
  • Learn about your options to roll over your vested balance, keeping in mind the tax implications if you receive a distribution.

    • Tax withholding information
    • Emergency contact information
